      I'm told that 2 COBOL programmers attempted to ship simultaneous releases on different branches in late 1986, rushed to deploy into production months before the code was done...and misplaced the punchcards which describe how to apply thermal paste around the custom coprocessor.
      This architectural flaw resulted in a cascade of failures partway through the v36 support window.
